The University of Waterloo is situated in Waterloo, Ontario, The university received its official flag on Friday, 19 November 1999, and although I have not yet been able to find an image of the flag, a complete description of it can be seen at the university website.
The University of Waterloo's newly created flag will be run up the flagpole at the campus entrance on University Avenue. David Johnston, UW president, will do the honours and raise the flag for the first time, assisted by Sgt. Wayne Shortt of the UW Police Service.
The campus flag, displaying UW's colours of gold, black and white, contains chevrons and three red lions similar to the UW shield. It will fly next to the Canada and Ontario flags at the reconstructed entrance to the university.

University Colours
The official colours of the University of Waterloo are gold, black and white.
University Arms and Motto
The Arms and Motto for the University of Waterloo, as first adopted in October 1961, and as officially granted in August 1987, by the Lord Lyon King of Arms, are described as follows:
Or, on a chevron Sable between three lions rampant Gules a Chevronel Argent. Above the Shield is placed an Helm suitable to an Incorporation (VIDELICET:- a Salade Proper lined Gules) with a Mantling Sable doubled Or, and on a Wreath of the Liveries is set for Crest between two maple branches in saltire a trillium displayed and leaved all Proper, and in an Escrol over the same this Motto "CONCORDIA CUM VERITATE"- In Harmony with Truth"

My colleague, who spent a year of his undergraduate degree as an exchange
student at the University of Waterloo, says: "I think there's usually
one [flag] flying next to the main entrance (alongside the Canadian and
Ontario flags). It may just have been the university crest on a white
background."

University of Waterloo.
